FMEA – Severity Assessment

Severity assessment of property failure

The definition differs for each type of FMEA analysis. Therefore, the type of analytical process must be specified (for individual failures or for properties).

FMEA type selection in AyMINE

The overview below is based on the standard for the automotive industry. Before use, ensure it is relevant to your case!

DFMEA

  1. Level 1: No effect
  2. Level 2: Perceptible, slightly annoying
  3. Level 3: Moderate annoyance
  4. Level 4: Highly perceptible and annoying
  5. Level 5: Disruption of secondary function
  6. Level 6: Loss of secondary function
  7. Level 7: Disruption of main function
  8. Level 8: Loss of main function
  9. Level 9: Non-compliance with safety regulations
  10. Level 10: Could endanger personal safety

PFMEA

  1. Level 1: Unnoticeable effect
  2. Level 2: Random fault does not disrupt operation; minor effect on the user
  3. Level 3: Some products need reworking, triggers a minor response plan; moderate user annoyance
  4. Level 4: All products must be reworked; the product has a significantly annoying effect on the user
  5. Level 5: Some products must be repaired, high risk of recurrence; product disrupts secondary functions
  6. Level 6: Production interruption of up to 1 hour, all defective products must be reworked; loss of secondary function
  7. Level 7: Products must be sorted, and some disposed of; production halts for more than 1 hour; product disrupts main function
  8. Level 8: Most production must be disposed of, production halts for up to one shift; loss of main/critical function
  9. Level 9: Threat to manufacturing regulations, threat to product regulations
  10. Level 10: Fault may endanger worker safety; restricted functionality affecting safety until service

FMEA-MSR

  1. Level 1: No effect
  2. Level 2: Minor annoyance
  3. Level 3: Moderate annoyance
  4. Level 4: High annoyance
  5. Level 5: Disruption of secondary function
  6. Level 6: Loss of secondary function
  7. Level 7: Disruption of main function
  8. Level 8: Loss of main function
  9. Level 9: Will not meet qualitative or regulatory requirements
  10. Level 10: Threatens safety
